Mueller grading of systemic sting reactions

Grades the severity of a systemic reaction to a Hymenoptera sting (Grade I–IV) by the most severe organ involvement.

Evidence tier: Expert consensus. A descriptive severity grading (not a validated predictive score). Use to classify the worst systemic reaction, not to predict future risk.

Formula / logic

Grade I: generalized urticaria/itching/malaise/anxiety. Grade II: + angioedema, chest tightness, GI symptoms, dizziness. Grade III: + dyspnea, wheezing, stridor, dysphagia, confusion, impending doom. Grade IV: + cyanosis, hypotension, collapse, incontinence, unconsciousness.

Interpretation

Higher Mueller grade and an elevated baseline tryptase both increase the risk of severe future sting reactions and inform venom-immunotherapy decisions.
